"Internal Server Error" on Form Submission

  1. I just added a form to my site at: http://technicallysimple.com/survey-online-productivity-course/ – when I fill out the form and click submit I get an "Internal Server Error". The form data does end up getting stored, but this error message comes up instead of the confirmation (I'm using "text" confirmation, but tried using "redirect" confirmation and got the same error).

    The other forms on my site, including the contact form (http://technicallysimple.com/contact/) work fine.

    Any idea what might be causing this error?

  2. Does the form in question work and submit fine from within the formbuilder preview?

  3. I haven't tried it in the Preview…but I discovered that if I disabled the e-mail notifications that the error goes away. So, at least I've honed in the cause. I'm going to leave the e-mail notifications off for now…until I have some time to investigate further.
