There's no need to be curt.. especially in your first post here. Often times these questions do get answered via email support and the threads don't always get updated accordingly. I'm not sure if that's the case here or not, but nonetheless I ran some tests and here's what I found.
I downloaded and installed the free jQuery Mobile WordPress theme I found here.

I activated the theme, tested it with several different forms and all of the forms displayed and worked properly. That said some of the advanced formatting in my test forms is not best suited for smaller resolutions, but the forms worked just fine in the mobile version of Safari.

So, if they're working properly here, it's possible that there is another issue at play. Most likely conflict with the theme or another plugin. Here's some useful information on testing for conflicts.
