JS Error with adding image to radio button

  1. Hi Team,

    I am creating an estimator for my site and its on the demo site now.

    I have added radio button in the initial stage and once that is done I am giving a conditional logic to display appropriate forms.

    Here is the first screenshot of the settings:

    Now I set the conditional logic at the section break:

    I check the preview:

    Till here its fine. The problem starts after this.

    I click on the radio button to show the conditional logic and nothing happens.

    I added the form to a page to check whats happening and there is no form at all.

    I have been through lot of threads to understand the issue and even deactivated all plugins and then even switched the theme and still the same issue.

    I noticed that there is a JS error as below:

    This is weird because the image tag is having issues. Not sure how to fix this and need your help.

    Hi Srinivas,

    You need to edit your radio button field and enable 'show values' and then give each option a value that doesn't include the html.
