Language questionnaire

  1. Hello,

    I have to build a typical "language questionnaire" for an english academy (we're in Spain). Here's an screenshot of the PDF I have to "transform" into a form: https://skitch.com/danielmt2k/8nddx/level-test.pdf-pagina-1-de-4

    That is, a lot of simple questions with one correct answer. The form doesn't have to make any calculations, just to let the user select an answer.

    I have searched a lot, but didn't find any sample to look at. Which is the best way to get a form like this? Any sample page?

  2. Daniel, you should be able to just use a radio field and place your question as the label. Then you can use our ready classes to have the radio fields inline if you want them horizontal:

  3. Some months ago, I found a page where there were a lot of sample forms. I don't remember if it was an official Gravity Forms page.

    Do you know of any?

    Yes, it worked. Thanks. I just wanted to have ideas to implement the form. I remember a page, where I could see over 50 forms with great ideas.
