  4. Checked again today and I too got the 404 error. Please post a URL to the live form and we'll give it a look. Please remember the madlib form is just a demo and not something that we're officially supporting. We'll be happy to give it a quick look and see if we can point you in the right direction, but beyond that, you may need to acquire some assistance from a qualified developer if you still have too many problems.

    I still have problems... with some of the text appearing like a letter

    Is it possible to have the Paragraph box?

    I also am trying to populate the name field with the name inputted at the top of the form

    using this I think it will work not sure of how I could repeat the process where the name would be used again in the text.

  6. You have a bunch of issues, but we should try to resolve one at a time.

    In Chrome, this line is not visible:

    "Report includes the following scores showing significant disadvantage:"

    Because the next line wrap of fields overlaps. It looks fine in Firefox, but it is still too tightly spaced.

    Can you try adding a paragraph around the text, or are those inputs supposed to be in the middle of that sentence? What are those three fields for?

  7. I was thinking if it would be possible to send the word Document this try to emulate. That way you could have a an idea of what I'm trying to do. I will put a paragraph around the thing you mentioned then post a confirmation of me doing that.

    Would this work? so the form is part madlib part normal gravity

  10. Joseph, what are you trying to get help with at this point? Are we still working on the original problem, or is that resolved and you are now trying to get help with adding a multi-select?

    You can make some parts of your form non-madlib. Add a custom CSS class to the fields you do not want affected, and then exclude them in your CSS so the madlib style does not take effect.

  11. Sorry to be annoying... my brain boings around quite a bit.

    1) I would like to get the mad lib form to look right in all browsers
    2) I would like certain fields to auto-populate based on the First Name and Last Name Fields. So the first name is used when referring to the person later. That way it doesn't need to be gender specific.
    3) I would also like multiple item fields that have reveal a number field, for each selection (if possible) so scores can be entered based on tests selected.

    I have excluded the madlibstyle but the multi select field overlaps the html text.
