I have 2 forms - 1 newsletter sign up and one - contact. Mailchimp works ok on the first, but I can't open the second form from Mailchimp "add feed" window. After I select list and form, nothing happens (just spinning circle). Am I doing something wrong?

No way to debug this without seeing it firsthand. If you can send us a WordPress admin login for this site via our Contact Us form i'd be glad to take a look and see what is going on.

The issue is the MailChimp add-on doesn't like the ( and ) characters in the field names for Phone (home) and Phone (cell).
I changed these field names to Home Phone and Cell Phone and the MailChimp add-on is working. I didn't setup the integration, so you will need to add the MailChimp feed and set it up.
We will look at seeing why this happens, but we would have to release an update to the plugin so the easy fix until we do is changing those field names as I have done.
