Multi-page form not advancing

I am using 1.5.rc1 and when you get to the second page of my form the Next button "Go to Payment Options" takes you back to the first page. After this occurs the Next button on the first page no longer works as well.

We have tried to debug it as best we can from just the frontend, we would need a WordPress admin login to try and debug things further as we were unable to determine the cause from the frontend alone. You can send us this information via our Contact Us form, reference this forum post and then we can take a look and see what is going on.
