Multiple column forms

    Hello. I am new to Gravity Forms. I want to create a form that has:

    1. Centered header at top.

    2. Three column format for about 8 lines of the form.

    3. Two column format for 4 more lines of the form.

    Hope someone can give me some insights on how to do this, thank you!
