Multiple "I agree's"

My client's online form requires sign off from 1-6 people due to the credit involved, so each checkbox needs to be labeled with a name.

    The basic aim is this:
DIRECTORS (At least one, and up to 3 people)
[ ] I, [ name ] have read and agreed to the terms and conditions.
[ ] I, [ ] have read and agreed to the terms and conditions.
[ ] I, [ ] have read and agreed to the terms and conditions.
GUARANTORS (At least one, and up to 3 people)
[ ] I, [ ] have read and agreed to the terms and conditions.
[ ] I, [ ] have read and agreed to the terms and conditions.
[ ] I, [ ] have read and agreed to the terms and conditions.

The names and their details are entered on an earlier page in the form. The ultimate would be to have the names carry over onto this section as text, and only show the lines applicable (meaning, if there is only one Director, there is only one agreement line under Directors).

But at this stage, I can't even manage to setup checkboxes followed by a text field sitting in text. 'terms and conditions' would need to be a link, as well.

Any suggestions on the best way to handle this would be greatly appreciated.

    Thanks.

  2. Hi Caz,

I would recommend using the gform_pre_render filter to retrieve the names submitted on previous pages (which would be available in the $_POST variable) and then dynamically populating checkbox values based on those names.

This is a intermediate-advanced customization so if you may want to consider hiring a developer to assist you.

    Thanks a lot, David.
After looking at that, I'll be looking at developers for that one. :)
