multiple product selection each having a quantity option.

    Hi. I've been struggling to create the following, please could you help me?

    I am looking to create a form that looks like this: http://www.rentacrate.co.za/wp-content/uploads/2012/09/Screen-shot-2012-09-17-at-1.45.12-PM.png

    Radio buttons to select the product/item, and next to it, having the quantity field where if the radio button selected will enter the quantity amount of the product.

    Could you please help?

    Do you need to have the radio button at all? Why not have a drop down field with the quantity, or a number field with a quantity of zero, where zero means "none" (the equivalent of not clicking the radio button) and a quantity means both the quantity and that they "checked" the radio button?

    Do you have an example online of what you have tried so far?
