Name Field Discription

I'm trying to add this test in the description are of a Name field. I was able to do it one of them but when I try to add to another name field, it keeps coming up as an error. Please list other team member & Shirt Size (First Name, Last Name, Shirt Size on one line)

Example:
John Smith, XL

There are a lot of conditional logic with this form. If you were to select an event, then another box pops open. If you were to select basketball, then two boxes open up. One box is where they can select an additional event. The other box is where the team captain who is registering, can enter the other team members and their shirt sizes. The descriptive text like what I posted is giving them instructions on how to enter the inform in the box.

Where I'm having troubles is when I use this with a name field. A lot of the events, there are singles and double and only one team member. This is where it's coming up with errors when I try to add below in the descriptive section of the box on the admin side however, I was able to add it to one of them but when I try to add it to the rest, bowling doubles, tennis doubles, etc, it errors.
