Need to recreate multi-column form with pre-defined content

    I need to recreate the form on this page for a client:

    http://www.gilbertelectricalsystems.com/products/reactors/rfq/harmonic_reactor/step01.php

    The closest thing I can find in Gravity Forms to do this is the list form, but I need multiple rows to be available without adding them, and I need the first column to be pre-filled.

    Is there any way to do this in Gravity Forms?
