New Purchase - Can't Create Forms

I just purchased the Gravity Forms plugin but when I went to try and create my firm form I don't think I'm getting the full creation template. As you can see in the screenshot link.

Can someone tell me why I get the error "Please Enter A Form Title" and how to resolve? I have a tight deadline to finish this site and this is a major road block.

Hey I've had the exact same probel - and still do.

But I found out of a temporary fix. Minimize your entire screen.

Zoom way out.

Then there will appear the option for entering a Title Name.
