Pardon my newbie stance, but I'm really at a loss here.
I've set up the following contact form to capture detailed information:

I have several custom fields set up, and none of the entered values are being captured
in the entries database. [I also have custom Notifications set up to email this info to two admin addresses.]
The custom fields use Conditional Logic. Would this have any effect on how those values are being captured?
For example, there's a question about an 'event planner' that triggers several more form fields. None of those are being captured.
Additionally, I can't get any data to populate a Highrise entry via the Highrise CRM.
Any assistance is greatly appreciated.
