Oops! We could not locate your form.

I have 22 forms currently on my new website (which is still under construction). All was working fine and then all of a sudden all 22 forms disappeared. The forms are still listed in the gravity forms plugin but when you click on a form and enter the form editor only the default "Untitled Form" field is shown, all other fields i entered have disappeared. When viewing the pages that the forms are supposed to be visible on i get the 'Oops! We could not locate your form' error message.

I am running the latest wordpress 3.4.1 (which i updated to after the problem occurred) and gravity forms 1.6.4.5. I have not installed any new plugins or themes too recently but just in case i've already followed your plugin conflict and theme instructions, with no luck. I've also looked over similar threads here and tried a few things with no success.

Creating a new form gives the same result, i can create fields but as soon as i save the form all fields disappear, so i can not simply recreate them all, some where also quite lengthy so i want to avoid this process anyway. At the time the error occurred i was simply adding some new pages with forms to my website. One minute everything was fine and then nothing worked.

    Any suggestions?

    Regards, Tim

  2. If you have not changed domain names or hosts, it sounds like something was upgraded or changed with your hosting provider. Have you checked with them? I've heard of hosts upgrading PHP and Apache without warning, which can cause trouble like this.

I called my hosting provider and they have guaranteed nothing has changed on their end. So what next? Should I reinstall gravity forms. I assume I will loose them all as exporting them returns a blank XML file.

Can you try to re-install Gravity Forms using FTP. Then can you go to the Settings page and add &setup to the end of the URL and see if you get any error messages?

Reinstalled using FTP. Still no forms but i do get the following error messages when following the steps you provided above:

WordPress database error: [Table 'wp_rg_form_meta' already exists]
    (CREATE TABLE wp_rg_form_meta ( form_id mediumint(8) unsigned not null, display_meta longtext, entries_grid_meta longtext, PRIMARY KEY (form_id) ) DEFAULT CHARACTER SET utf8)

WordPress database error: [Table './nation5v_wordpress/wp_rg_form_meta' is marked as crashed and should be repaired]
(SHOW INDEX FROM wp_rg_form_meta WHERE Key_name='form_id')

    What does this mean?

This is probably a silly question. But how do i use this tool? Is there some documentation somewhere because i can't find any.

its ok i figured out how to use it. But it just says "This form's meta could not be recovered" for all 22 of the forms. I can still not create any new forms either?

  10. This is your problem:

'./nation5v_wordpress/wp_rg_form_meta' is marked as crashed and should be repaired]

The MySQL database table is corrupt and needs to be repaired. You should contact your host for support on how to proceed. You might be able to repair the table using SQL commands in a tool like phpMyAdmin, but you should contact your host to be sure. You may need to restore this database table from a backup if the repair fails.

Ok, will try that. Out of interest what would cause this to happen?

  12. There are lots of things that can cause corruption in a database. Power fluctuation, power outage, bug, randomness, hardware failure, improper shut down, etc. Sort of like when a hard drive fails on a PC. Most of the time it can be fixed easily, but it pays to involve the host so they can be sure it does not happen again.

Just called my hosting provider and repaired the corrupted database. I can now create and save new forms which is good news. But the original 22 still return the "Oops! We could not locate your form" error message, although they are still all listed in the form editor. Can these forms be easily retrieved somehow or is it going to be simpler to recreate them all? Unfortunately being the slow learner i am i did not export a backup of these forms.

Apologies. The 22 forms have returned. All the fields for the 22 forms are now visible in the form editor. I just have to re-enter the shortcode onto the posts and pages and all works perfectly.

Thanks for the help guys, i know you must get these sort of questions all the time. top notch technical support.
