    Something is introducing a break tag into your form markup that shouldn't be there. If you remove that, the layout works as designed.

    I would assume it's some kind of conflict with either your theme or another plugin. You'll want to test for conflicts to try and narrow it down.

    I figured out its a problem with the theme. What can I do resolve it?

    Sorry, I can't tell you that for sure. Something in your theme is adding the markup.. probably some script that's included via the theme's functions file. You'll need to contact your theme developer to help you track down the source and find a solution.
