    I am looking for a solution that lets me use 1 form with multiple input fields as follows:

    form field 1
    Content on WP page
    Form field 2
    Content on WP Page
    Form field 3
    Content
    Submit

    Is this possible and if so how

    The only way I could see to do this would be to use our HTML blocks and include the content you want between fields in our HTML blocks.

    The HTML field is one of the Standard field types in the form builder (first section which is normally open when you load a form to edit.)

    You an insert that type of field into your form, and then put your HTML into that field, which will then be used in the presentation of the form. Here is the documentation for that field type: http://www.gravityhelp.com/documentation/page/HTML
