I can't duplicate this issue, but a client of mine has done so multiple times. When filling out a paged (6-page) form, at the end of page 2, he hits Next and it redirects back to the first page with none of his info in the fields. He's gone through it several times with the same result. He's using Safari (as am I).
Any ideas as to why this might be happening?
Do you have a link to this form? If it's only happening on his machine, it could be something related to his network.

Just got an email from someone else with the same issue. Aggghhh! Losing prospective members. This is a looooong form so they don't want to try refilling it when it wipes clean.
The form is located at http://globalministriesonline.com/join/application-form/online-application-form/ . There are two others that are used less frequently that I'm now concerned about, as well. Any help is appreciated!
Well, I got through it ok (i didn't submit though). I did notice this in your source: Screenshot Not sure if that has a potential conflict in it, do you know what that is in reference to? I'm assuming it's something theme-related.
