I'm not sure, I'm not able to re-create it as well. The only difference that I can notice is the user trying to input the form was using IE8 versus me trying both Chrome 24.0.1312.57 and IE9.
I did visit the user though and verify several attempts and the only field highlighted was the phone field in red saying invalid field format (or something along those lines, had I thought to print screen I would have).
Its the first 800 number we've had inputted into the form, but as you said I can input just fine on my machine.
I guess I'll just chalk it up to lack of caffeine and gremlins.
