For some reason my forms are no longer creating posts or sending an email notification upon completion. I checked and both are still set up correctly. What is the typical reason for this?

Can you post a link to one of your forms? It sounds like they may not be submitting at all. Are entries being created when a form is submitted? Something on your web server may have changed.

there's a link to one of the forms. they are being submitted, because i can see the entries in the gforms dashboard. they previously worked, so i'm not exactly sure what's happening. if you're up to looking at the back end, please send me an email patmac1@gmail.com

If entries are being created then the form is being submitted. The first thing you need to do is check for theme and plugin conflicts by following the instructions here:
http://www.gravityhelp.com/documentation/page/Testing_for_a_Theme/Plugin_Conflict
If Notifications are not being sent then most likely this is a web server issue. But start with testing for theme and plugin conflicts.
