previously selected "selections" are clearing out

    Hello, I have a page (listed below) that i have inserted a form containing a SUBMIT button which displays based on conditional logic.

    When First and Last name are entered into the form, the previous Radio Button selection(s) clear out causing the SUBMIT button to disappear...obviously because the conditional logic is no longer met.

    I'm not what is causing the Radio Button selections to clear out after First & Last Name with number or email are entered.

    Thoughts?

    Sounds like you might be running into a recent bug with Chrome's Autofill. Is that the case for you here? This is getting corrected by their team, see: https://code.google.com/p/chromium/issues/detail?id=244645

    Yep... you nailed it. I just tried two different browsers and it works fine.
