The quantity field for one of my products disappeared, so I removed that entire product field and added a new one to replace it. It displayed after that, but then the quantity field for a different product disappeared. So then I removed and readded that product. Now the original quantity field that was disappearing is gone again. WTF? Any ideas?

Please provide a link to the page on your site where the form is embedded. Also, does this happen in the form preview as well as the public page on the site?
When did the problem begin?
Are you using any conditional logic in the form?
Have you already checked for theme and plugin conflicts using these instructions? http://rkt.gs/testing
