Really dumb question - how do I add my form to a pre-existing page?

    I feel really dumb, but I have made a beautiful form that I only know how to view in preview mode!

    How do I add it to a page? Or does it have a URL or post ID that I can link to? I only can find the preview URL!

    Please be very specific!
