Required fields are not working

I have created a form today, but somehow two required fields are not working.
The first is for filling in fist + last name
The second is the address field

After filling in these fields and the rest of the form, they show up after submitting the form as empty required fields.

    Please help
The form can be found here http://www.fitmagazine.nu/adverteren/leverancierindex/

  2. Looks like the validation is working, as I cannot submit the form without the required fields being filled in. So, it must be something causing the information to not be saved after submission.

If you submit the form from the preview, is the information saved in all the required fields?

If so, then you should begin by testing for plugin or theme conflicts http://rkt.gs/testing

  4. Have you testing for theme and plugin conflicts?

How are you changing the language on your site?

Any other custom code in your theme's functions.php that would affect this form or all forms?

the problem occurs when plugin 'adrotate' is activated
    (http://www.adrotateplugin.com)

How can we solve this issue, because I also want to use the 'adrotate' plugin

  6. The conflict has been reported before with the adrotate plugin. I'm not certain what the error is that causes the problem. Can you contact their support and see if there is a resolution?
