Sample Forms?

We do not currently have sample forms for download. It's something we'd like to include in the plugin itself when creating a new form, but we've been focused on other product development and it's not high on our priority list right now.

The forms are whatever you want them to be. It's wide open. You have a large selection of different field types you can use so just add the fields you need to capture the data you want to capture. Forms are very custom to the use case and user requirements, so lots of people handle things differently.
