Specific Tables needed - can Gravity do this?

    I need to recreate this form (multiple times with minor revisions) on a WP site. Can Gravity do it exactly like this or better? Hard to tell from the small screen shots. This is more of a survey and can't change the content.

    No, Gravity Forms doesn't currently have this field type. It is a field type we plan on adding in the future.
