I use the beta Survey ADD-on.
There are a few problems with it.
1. When having the survey likert in a form and AJAX is true, you can't check an option.
2. When you have a FORM with survey LIKERT with MULTIPLE ROW's and with PAGE-BREAKS the selection on the multiple rows ar not remembert/saved.
Can this be fixed somehow?

I confirm that's page break element: without page breaks, the complete survey with a lot of likert with multiple rows works perfectly.
By adding page breaks, likert values aren't saved, and on the survey the radiobuttons aren't clickable.

Do you have AJAX enabled the form? When i turn on the AJAX enabled i can't either click the options on the likert.

Thanks, the likert with MULTIPLE ROWS does work now, even with AJAX enabled.
Almost every survey option works, but the RANK isn't.
When AJAX enabled you can't rank the options. Also when you have page-breaks and set an RANK.. you ga to previous page en back again the options you ranked are gone.
I don't use the RANK option, but i thought i should mention it to you.

Correction; AJAX ENABLED still doesn't work correct with PAGE-BREAK. At first you can choose the LIKERT options, but when you go to the next page and back again you can't re-select the LIKERT options.. it's not responding.
