Tab Slide out Plugin to work with Gravity forms

    I was told to post in here by one of the admins since then my problem has changed... Basically I'm trying to add a gravity form to slide out on every page... I'm a rookie. I tried to do it through Jquery had no luck since then i have come up with this....

    In the option menu its pretty simple it gives you an option to customize the template of what you want the content of the slider to be pages, posts, videos, pictures, and Widget area... I have set it to be a widget area then i simply go into my widget options add a gravity form widget to the tab slide out box it gives you... East right!? I cant get the form in the slide out tab looking exactly how i want but the form won't function! I have tested the form in a normal widget area on my homepage and they work fine, so there is some type of conflict!
