This is an agreement form, where people have to check 'agree' to the terms and conditions. I've found that if I use Chrome autofill (for my name, email etc) and hit 'enter', the 'agree' checkbox unchecks itself. Other people have told us they've had an issue submitting the form, and I suspect this is what the issue was.
Any work-around for this?
