When I'm trying to add a start/end time for my form, it shows me the month & year, but where I think the calendar would be showing up, there is just a thin outlined box.

It appears that you've got a styling conflict originating from your theme or another plugin. I can't tell you much more without seeing the actual form and inspecting problem. Can you post a URL to your form page? If so, I'll be happy to take a look.

Well, I looked at that form page and don't see any date fields.. am I missing something obvious here or is there another form I need to look at?
