title of form not showing up on page form is assigned to

    Hi
    I just made a new form and the title ( of the form ) is not displayed on the page like my old forms. I'm using the default template for wp- and if I click http://auburn.adszoom.com/?gf_page=preview&id=64
    preview I can see the form and title of form but on the page Its not there, The title of the page http://auburn.adszoom.com/adszoom-com-email-campaign-management-weekly-2-0-member/
    is displayed correctly and such but the title to the form is not? Was there a change to GF V 1.6.8.1 ? I called server host and had data base repaired just in case.. No luck
