I've created a form on my "contact us" page called "inventory updates" which adds the submission to our MailChimp mailing list. I'd like to ad an option (using conditional logic) for the submitter to share our site with a friend. I'm fairly sure I have everything set up correctly for this but it just isn't working. Please help! I have read several post on the forum including the "best way to create a tell a friend form," but it just isn't working for me.

Here's the html (I've never used PasteBin before so hopefully this works).
Please export this form (Forms > Import/Export > then the top center menu item "Export Forms") and email me the XML file, so I can install it and see how you have configured it. It does not sound like you're using any custom code right now, just the form?

Tyler, I had no problem sending the "tell a friend" email when I imported the form on my site. Here is a screenshot of the email I sent to myself as a friend:

However, when I tried it on your site, I had a couple problems. One was that the Yes/No field for "Would you like to tell a friend about us?" requires a unique answer. Remove that restrictions, otherwise you will get at the most one Yes and one No. That is not a place that should require a unique answer.
The other problem was that I could not submit without checking the box "Yes, I would like to receive inventory updates!". Maybe that's how you intended this to work, but it's not intuitive. Maybe I don't want to receive updates, but I just want to tell a friend about your site? Maybe I already receive updates?
In any event, the form worked on my site. If the form submits fine on your site (no validation errors) but the friend does not receive an email from you, then you have an issue with the notification emails being sent by your server.
