Two options one line

    I am creating a form for a Church client. They are looking to have options of where to give and how much. I can not find a way to keep the where and how much in line????
    Something like this:
    Church Offerings
    Tithes $
    General Offering $
    Benevolence $
    Also, is this where the pay pal addon would work well?
