Unable to add new form fields or save form

I'm trying to create a new form on a development subdomain where the webhosts are using PHP 5.1.6, and so I have to use WP 3.1

I'm unable to save a new form, or add fields. There are no plugin conflicts (GF is the only plugin for now), or theme conflicts (developing from twenty ten), and I'm told the uploads directory is writable. I'm stumped. (and frustrated by a webhost not running latest versions). Any clues?
