Unable to create for due to overlap wordpress menubar

Hi,

We are unable to create a form, the pop up for creating a form shows benieth the wordpress menu bar in the admin.

We only see create form. We can't place anything in form title, because its behind the wordpress admin bar.

Please help, We dont want to mess with the code.

This is due to a theme/plugin conflict. All you have to do in this case is enable "No Conflict Mode" on the Gravity Forms Settings page.
