Unable to edit name field format

    I am unable to edit the name field format drop down in the form edit screen, see here:

    When I attempt to use the dropdown, nothing happens.

    The last time I saw this happen, it was in a product field, but the issue was the same: could not use the drop down. Can you delete this field from the form and add another name field in, and see if you can use that drop down? If not, please check for theme and plugin conflicts by following these instructions http://rkt.gs/testing
