Unable to see form fields

    When I create a new form all I see is the name of the form - the rest of the page is blank. I am unable to see any of the fields.
    I desperately need some help.

    It sounds like a plugin or your theme could be including scripts on the form editor page, go to the gravity forms settings page and switch No-Conflict Mode to ON and see it that helps if it does it would confirm a conflict. Either way you should also test for a plugin/theme conflict by following the advice on the following page, before you start you should switch No-Conflict Mode back to OFF, http://www.gravityhelp.com/documentation/page/Testing_for_a_Theme/Plugin_Conflict
