Unable to view the left side of the edit fields within the form editor...

Hi, if anyone can help, I'm unable to view the left side of the edit fields within the form editor...

Everything is visible until I click "edit" on a form field....

The same thing happens on PCs and Macs, firefox, safari, chrome...
the site is running WordPress 3.3.2,
It could well be something with my Wordpress theme, or is the new version of WP interacting funny with the plugin?

Thank you, it's the theme... since i don't want to have to re-populate the theme widgets, I'll just create the forms in a dummy installation and import them into the real one.
