I have a form with 3 file upload fields (1 is not always used). They usually work with no issue but occasionally one or more of the files uploaded will be overwritten by a previous file attachment. We even have text telling the user to name the file in a specific way to try to avoid have files with the same names.
This has happend 3 different times in the past year and it seems to happen when there is an update available for Gravity Forms.
