    This form functioned perfectly the first few days after I posted it.

    Now, when the form is filled out and submitted, a duplicate form appears below the original.

    The data entered by the applicant is not recorded.

    I submitted your form without entering and data, and the page does not appear to do anything. I don't think your form is submitting at all.

    Does the form work in the form preview? If so, please proceed by checking for theme and plugin conflicts by following these instructions http://rkt.gs/testing

    Also, you can try simply going to the permalinks option page, and just saving your existing permalinks to update the .htaccess file.
