I am having an issue with my form. I would like to send my website privately.
Essentially the form is not loading when I enable conditional logic and I am seeing this error
"TypeError: window.gf_form_conditional_logic is undefined" on line 21.

I am running WP 3.5 and GF 1.6.11.
From what I can see jquery ( v1.8.2) is only being loaded once in the head section.
Before we get to your site information, can you try the following steps please?

I ran some test.
The conditional logic works when I preview the form.
The conditional logic works when I use the default theme.
The conditional logic does not work when de-activating all other plugins.
If the conditional logic isn't working when your selected theme is active, then this means there is some sort of JS conflict or error. Can you check to make sure there aren't any other JS errors, and make sure JQuery and all Gravity Forms JS files are being called in your header?
I am not seeing any JS errors in my console.
The following JS are loaded in the head:
jquery ( v1.8.2)
gravityforms.js?ver=1.6.11
conditional_logic.js?ver=1.6.11
So when the page loads my form is hidden. If I set the form to display and then click on my radio button which is supposed to trigger some conditional logic I receive this error:
"TypeError: window.gf_form_conditional_logic is undefined"
