Using a form on the 404 page of a site

  1. I'd like to have a form on my 404 page which would allow the user to enter some info and send it to the webmasters. I'd like to requested URL (ie. the 404'd one) and the referring URL to be entered automatically. Then, I'd have two additional options fields (name and email).

    How can I get these to be pre-populated? I see where I can enable dynamic content in a field, but I am not sure how I would get this to actually work.

    Also, since I'm using WP SUper Cache, I am not sure if this will even work?

  3. Good to hear that you figured out a way to do it. Yes, the referrer is one of the dynamic variables you can use to populate the value of a field (or hidden field). Very handy! Not a lot of people realize how many variables are available.
