    We are using a plugin to create a duplicate site on a MS network from an existing one. Upon creation of a new site in WP MS - the form gets 'duplicated'. So when we should have 1 form, it actually creates two of the same forms with different ID's. Any ideas?

    I'm sorry, I have no knowledge of that plugin, and as such, I can't say why it would double the form...
