1.7.2 Cannot add new form, "title" field off-screen

    Hi, the latest version of Gravity Forms has added a "new form" popup, but the title field is position too high - off-screen - so it's impossible to fill it out and create a new form. I tested on Win 7 in FF20, Chrome26 and IE10. See screenshot here: http://ruffinsolutions.com/gravity-add-new.jpg
