Can I have you start with this:

Also, can you duplicate the form with an issue, and let me know if that duplicated one works fine?

I tried to do what you wrote (change theme, deactivate/activate plugins, duplicate form).
    Not working...

So you have a single form that fields can't be added to. Are you using the latest versions of Gravity Forms and WordPress? The error still occurs when using Twenty Eleven and no other plugins enabled? Is there anything special about the form?

On every form, or just the main troublesome form? If it is on every form, then it is most likely a memory issue, but I wouldn't expect to see a 403.

I have just one form, it's happening in the main form and in the main-duplicated form.
I haven't tried on the other forms, it takes long time to build another, should i create a new form or it is memory mistake?

Yes, please just make a random form with as many items as possible to check. Twenty-six input fields.

Can you export the problematic form and send it to peralty@rocketgenius.com so I can try it on my test environments?
