Another display:none issue - 2011 theme and no other plugins

    I've tried for a couple hours to figure this out. Granted, I'm not very good at debugging javascript... so I'm hoping someone can help me.

    I think I've ruled out theme conflict... switched to 2011 theme for testing.
    I think I've ruled out plugin conflict... disabled everything except GF and the Auth.net addon

    The form on this page is still not showing because of display:none in the form div:
