Are there Form Samples/Templates?

    Are there form template samples e.g.. I want to build a form simlar to the first Contact Us sample on http://www.gravityforms.com/features/styles-layout/

    Im not quite sure how to customise the form this way - any help appreciated or tutorial on how to do this please.
