I've just been told that there have been complaints from certain users that they get a blank page upon submitting the form, and the data didn't get submitted.
It works fine with my Mac. The form is here:

Do they get a blank page as in the theme itself doesn't load.. white screen, or the theme loads the page just fine and there is no confirmation?
I can't recreate the issue so far, any word on which browsers these reported issues are occurring in? I would guess it's server/host related issue that happens periodically, but we would really need more information to be able to give much help beyond that.
