Certain Forms not showing up

    My site: bestdamnshortsaleexpertinportland.com has SOME of the forms show up, but not all. On the front page in the bottom right the form shows, but when you go to this page: http://bestdamnshortsaleexpertinportland.com/get-help-now/ the form that is supposed to be at the bottom of the page is not there. Same with the form on this page: http://bestdamnshortsaleexpertinportland.com/help-for-agents/submit-a-short-sale/.
    I know that these forms worked previously and I haven't done anything to change them. Any ideas?

    On those two pages you have a JS error showing:

    That error is not on your home page, so I'm banking on that being your issue, which looks like the Viperbar plugin.
