Check Box list not formatting (Label on line below checkbox)

  1. I have reviewed several posts, but am not able to find anything that solves my issue. I am using wordpress with the Inspiration theme. I have a list of checkboxes, but the label shows up on the line below the checkbox. The form is here: http://www.ofelosgroup.com/copavidamedspa/book-an-appointment/

  2. The problem you're seeing is related to the Uniform.js script that's being employed on your forms. This isn't default Gravity Forms functionality or something we support. The inputs are being replaced with graphical images and are inheriting from the Uniform CSS. I've seen Uniform used before but usually this layout issue isn't a problem.

    Since this is a theme/script related issue, you'll want to discuss this with your theme developer and see if they can help you with the styling issue or either disabling the uniform.js script.

  3. Does it make a difference that when I uncheck "Output CSS" from the GF settings page the problem goes away, but the other boxes (name, etc) look bad?

  4. The problem is a conflict with the uniform.js css. If you disable the Gravity Forms CSS then it may resolve that conflict, but then the other elements may not be styled properly.

    How does your form look in the preview page without the uniform.js script being added? Do the actual checkbox inputs line up properly?
